Why Your Clinic Ranks Fifth and the Practice Down the Street Ranks First

You know your clinic is good. Your patients say so. Your reviews say so.

So why is a practice three streets over sitting at number one while you’re below the fold?

Google isn’t broken and they’re not paying for it. Local ranking runs on three factors, and none of them measure how good you are at dentistry. Once you know what they are, you can look at your own listing and see exactly where you’re losing.

 


 

The three factors

Google ranks local results on proximity, prominence and relevance. That’s the whole system. Everything else is a version of one of those three.


  1. Proximity: how close you are to the searcher

When someone searches “dentist near me,” Google shows clinics close to them. Not the best clinics. The closest ones that meet a reasonable bar.

That practice down the street might be beating you on a search you were never going to win.

What you can’t change: your address.

What you can change:

  • Your service areas. Most clinics never set these. If you draw patients from four suburbs and your profile lists one, you’re invisible in three.
  • How you check your ranking. Searching from inside your own clinic shows you a result nobody else sees. Google centres it on your exact location.

How to check properly:

  • Open Google Maps on desktop
  • Search your treatment plus your city, for example “dental implants Mississauga”
  • Zoom the map out to cover your whole service area

Or ask a patient across town to search and screenshot it. That’s your real ranking.


  1. Prominence: how established you look

This is where most clinics lose, and where you have the most control.

Reviews matter in three ways:

  • Count. A clinic with 240 reviews signals more established than one with 18. This is usually the biggest gap between you and whoever ranks above you.
  • Recency. Forty reviews from 2019 are worth less than fifteen from the last six months.
  • Consistency. A steady trickle beats thirty in one week then silence.

Go look at the clinic ranking above you. Count their reviews. Check the date on the newest one. That comparison usually explains the ranking gap on its own.

Citations. Google cross-references your clinic against directories, association pages and board registries. What matters is consistency. If you’re “Smile Dental” on one site and “Smile Dental Clinic” with an old suite number on another, Google gets less confident. Less confidence means lower ranking.

Profile activity. A profile with recent posts, photos and answered questions looks like a business someone is running. One untouched since 2021 looks like it might be closed. This is almost free to fix and almost nobody does it.


  1. Relevance: how well you match the search

Most clinic listings are far vaguer than they need to be.

Primary category. You pick one primary and several secondary categories. The primary carries far more weight. If you do a lot of implants but your primary is “Dentist,” you’re competing with every dentist in your city for every search. Be honest, but be specific.

Services list. There’s a section for individual services and most clinics leave it blank. List every treatment using the words patients actually type. Not “restorative dentistry.” Write “dental implants,” “root canal,” “Invisalign,” “teeth whitening.”

Your website has to agree. If your listing says implants but your site mentions it once inside a general services page, that’s a weak signal. A dedicated implants page, with the treatment in the title and your city mentioned naturally, is a strong one.

Your profile and your website need to say the same thing.

The 20-minute self-check

Open your Google Business Profile and go through this list. Count how many you can’t answer yes to.

Proximity

  • Are your service areas defined and do they cover everywhere you draw patients from?
  • Is your address exactly correct, suite number included?
  • Have you checked your ranking from outside your clinic?

Prominence

  • How many reviews do you have versus the clinic ranking above you?
  • Is your most recent review less than a month old?
  • Are you responding to every review, positive ones included?
  • Is your name, address and phone identical everywhere online?
  • Have you posted to your profile in the last 30 days?
  • Are your photos from the last few months?

Relevance

  • Is your primary category as specific as it honestly can be?
  • Have you listed every treatment in the services section?
  • Does your description mention your main treatments and your city?
  • Do your highest-value treatments have their own website page?
  • Is your Q&A section populated?
  • Is your booking link connected?

Whatever you couldn’t answer yes to is your gap. That’s almost always where the ranking difference comes from.


Fix these three first

  1. Complete your profile. Primary category, every service, full description, photos, booking link, hours. Free, takes an afternoon, highest return of anything on this list.
  2. Build a review habit. Not a push for thirty this week. A system where every patient gets asked, consistently, forever.
  3. Give your best treatment its own page. Pick the treatment you want more of. This is what lets you rank for something specific instead of competing for “dentist” against everyone.

The clinic ranking above you probably isn’t better at dentistry. They’re better at being legible to Google.

That’s good news, because being legible is a solvable problem. It’s a set of settings, a habit around reviews, and a website that clearly says what you do and where.

It doesn’t require you to become a marketer. It requires someone to do it properly once, then keep it maintained.
